as possible, you can fill out the necessary paperwork ahead of time. We
have attached the usual medical and personal information forms below.

Do not worry if there are some things you do not know how to answer. One of our reception staff will go through these with you. Some of the things we find are forgotten are the list of medications that you are taking, and details of your dental or extended health insurance. We sometimes need your B.C. Medical number as well.
